Maternity Care Coalition (MCC) is seeking a Bilingual HFA Case Manager (Advocate IV) to deliver in-person, strength-based support to expectant parents and caregivers through the Healthy Families America (HFA) model.

What You’ll Do:

  • Provide weekly in-home visits to families navigating challenges like trauma, housing insecurity, or child welfare involvement.
  • Use your language skills to ensure families understand their resources, rights, and parenting goals in a language that feels like home.
  • Support family well-being through evidence-based parenting and health education.
  • Create Family Service Plans, make referrals, and track progress through case notes and systems like ETO and SharePoint.
  • Represent MCC at community events, family meetings, and social groups that foster connection and support.

Key Requirements:

  • Associates, Bachelor's, or master's degree in social work, Public Health, or related field.
  • Five years of direct service experience and a strong understanding of maternal and child health is a plus.
  • Bilingual fluency in English + Spanish or Haitian Creole (speak, read, and write).
  • Ability to work in-person and travel throughout Berks, Bucks, Delaware, Lehigh, Montgomery and Philadelphia counties.
  • Willingness to work non-traditional hours, including evenings and weekends as needed.
  • Valid U.S. driver’s license and clean driving record.
  • Comfortable working independently

  • Tech-savvy with the ability to document and manage family files digitally.
