The social worker works closely with the patient/caregiver's and other team members to assist the patient in identifying and utilizing appropriate community services. The social worker will act as a liaison between the family, staff, community institutions and agencies to meet the needs of the patients. The social worker coordinates the bereavement program for Hospice and provides grief and loss support to Hospice patients and their caregivers. Qualifications What is required... Completion of an accredited Social Work program with a Master's degree 1-2 years' experience in a healthcare setting, preferably with home health and Medicare experience; Current BLS for Healthcare Provider card (maintenance required); Valid Washington State Driver's License; Reliable transportation & valid automobile insurance.
