*Key Responsibilities:*
* Conduct comprehensive assessments to determine the care needs of patients.
* Develop, implement, and evaluate individual patient care plans in coordination with caregivers and physicians.
* Provide intermittent skilled nursing care to pediatric and adults patients at their residences.
* Supervise paraprofessional staff and licensed practical nurses, ensuring compliance with Colorado State law.
* Maintain accurate documentation and process orders, admissions, transfers, and discharge paperwork.
* Ensure ongoing communication with all disciplines involved in patient care.
* Adhere to federal, state, and agency policies and procedures.
*Qualifications:*
* Graduate from an accredited school of nursing with an associate or bachelor’s degree in nursing.
* Active RN license in the state of Colorado.
* At least one year of nursing experience in home health care, with pediatric experience preferred.
* Knowledge of state and federal home health agency regulations and compliance standards.
* Current CPR certification from an American Heart approved course.
* Valid driver’s license, reliable transportation, and active auto insurance.
* Ability to perform physical assessments and provide care in non-wheelchair accessible homes.
* Successful completion of a background check.

Pay: $70,000.00 - $80,000.00 per year
